VYRCO

Failure, symptom and evidence log

Separate observations from hypotheses and preserve a verifiable timeline.

Who it is for and what is included

Technicians, supervisors and engineers investigating failures.

Level: Basic. Includes instructions, recording fields, a fictional example and reference sources.

How to use it

  1. Assign an event ID and record time with time zone, equipment, mold and operating condition.
  2. Describe what happened, where and when. Transcribe alarms exactly and identify the source; retain original files.
  3. Keep hypotheses separate. For each authorized check, record method, result and evidence.
  4. Distinguish temporary containment, confirmed cause and closure. If evidence is missing, state pending validation.

Fictional example

FICTIONAL: EVT-DEMO-02, 09:15 UTC; interrupted ejection and an alarm recorded as AL-DEMO. Cause unknown. Hypothesis: mechanical interference, unconfirmed. Evidence: photograph and alarm log. A time correlation is not declared a root cause.

Scope and limitations

Does not diagnose automatically or authorize tests. Protect personal and confidential information before sharing evidence.
